Worker Letter

Many of you may be aware of the impending restructuring of Self Direct services into a program called Financial Management Services (FMS). At this point, FMS is scheduled to go into effect on November 1, 2011. While this does mean a change to how the program operates, we would like to assure you that there will be no change to the level of service you are currently receiving through our agency.

Below, we have answered several questions about what this means for you and listed some additional steps to make your transition to FMS goes as smoothly as possible:

FMS Contracts –FMS requires several contracts be signed and returned to our agency in order to provide payroll services. Please be advised that NO payroll can be issued until all contracts have been received from you and your client, so please sign and return them in the enclosed self-addressed stamped envelope, as soon as you receive them.

  1. EMPLOYMENT SERVICE AGREEMENT – FMS employees will be required to sign a contract between them and All Saints Home Care. A copy of this contract is enclosed.
    1. Please print your name on the front of the contract in the highlighted area.
    2. Sign and date on the backside of the contract in the highlighted areas.
    3. Send signed contract back to us using enclosed envelope.
  2. FINANCIAL MANAGEMENT SERVICES AND DIRECT SUPPORT WORKER AGREEMENT – FMS clients will be required to sign a contract between them and each of their employees. Copies of this contract have been mailed to the client by our agency.
    1. Both you and your client should print your names on the front of the contract in the corresponding highlighted areas.
    2. Both you and your client should sign and date on the backside of the contract in the corresponding highlighted areas.
    3. Send signed contract back to us using their enclosed envelope along with client’s Financial Management Services and Customer Agreement.
  3. FINANCIAL MANAGEMENT SERVICES AND CUSTOMER AGREEMENT – FMS clients will be required to sign a contract between them and All Saints Home Care. A copy of this contract has been mailed to the client by our agency.
    1. Have client send signed contract back to us using their enclosed envelope along with the Financial Management Services and Direct Support Worker Agreements.

Employee health insurance – All Saints will continue to offer AFLAC supplemental insurance at the group rate to all FMS employees.

Employee pay rates – Although it’s not official, it is our understanding that employees will be paid between $9.24 and $10.15 per hour depending on clients program, for all services provided on or after November 1, 2011.
